Step 1: Anticipate your arrival to a door slightly before someone else may be leaving or coming in behind you.
Step 2: Hold said door open while at the same time encouraging the other individual into or out of the building/room.
Step 3: Make eye-contact with the individual and offer them a genuine smile.
